Evolution Industries Warranty & Return Policy
Return Policy
All returns to Evolution Industries Inc. MUST be accompanied by a Return Authorization number (RA#). No returns will be accepted without this pre-authorized number. An RA# number can be obtained by calling Customer Service at 888-839-6213. Please be able to provide the following information when calling:
- Company Name
- Patient Name and/or Part Number
- Reason for Return
The issuing of an RA# does not guarantee a credit will be issued.
Warranty Returns
Evolution Industries, Inc. warrants all of its products and services, to the original purchaser to be free from defects in materials and fabrication. This warranty applies when the products are used as intended, without modifications, following the procedures for use as specified in the patient and prosthetist instructions.
Custom orders are fabricated as requested by the customer. Custom orders may be returned for evaluation, re-fabrication or replacement, however, custom orders may NOT be returned for credit. Evolution Industries, Inc. will take all reasonable steps to insure custom orders are fabricated to fit the customer’s requirements. Custom orders that are outside our normal manufacturing procedures are not subject to the warranty return policy, and will not be replaced under the warranty return policy.
Custom and Origin liners are subject to a 6 month manufacturers defect policy. Aura sealing sheaths and Origin sleeves are subject to a 3 month manufacturers defect policy. The warranty period is determined by the date the product is shipped from the Distributor, or by Evolution Industries, Inc.
All warranty returns must be returned within 30 days of the issuance of the RA# to be valid.
Stock Item Returns
Stock item returns will be evaluated to determine if they are in resalable condition. All items must be returned within 15 days of the issuance of the RA# for the RA# to be valid. Stock item returns are subject to the following restocking fees:
10% - Is in salable condition
Returned within 30 days of purchase date
Does not require repackaging
15% - Is in salable condition
Returned within 30 days of purchase date
Requires repackaging
OR
Is in salable condition
Returned after 30 days from the date of purchase, not exceeding 3 months of the original date of purchase.
No stock item returns will be accepted on items beyond 3 months of the original purchase date.
Special Orders
Special orders are not considered stock items, and may not be returned for credit. Special orders may only be returned for manufacturers defect warranty evaluation. Examples of special orders are as follows:
- Liners that are requested to be tacky on the inside
- Liners that are requested to be silk on the inside
- Origin liners and sleeves ordered in a custom color
